Insurance

Larchmont Physical Therapy works with many different insurance companies. As a courtesy we will bill your insurance company on your behalf. We will verify your benefits prior to your receiving treatment and will let you know what your financial responsiblity (if any) will be. You will ultimately be responsible for annual deductibles, co-pays, co-insurance and any unpaid charges not covered by your insurance company.

PPO's, Medicare and Workers' Compensation Insurance

California insurance companies require that you have a prescription from a physician to be reimbursed for physical therapy. This prescription usually comes from an orthopedic surgeon but can be obtained from any doctor such as your primary care physician. If you choose not to see a physician, but do have a diagnosis, we can treat you at a cash rate. Please keep in mind that if you choose this path we may request you see a physician at some point to rule out more severe or unusual causes of your symptoms and/or pain.

PPO/In Network

Larchmont Physical Therapy is an in network provider for Blue Cross, Blue Shield, Medicare, Motion Picture, Screen Actor's Guild, Director's Guild, Worker's Compensation, and many others. If your plan is not mentioned please contact our office (323-464-4458) and we will gladly verify your coverage and benefits.

HMO's/Out of Network

Unfortunately Larchmont Physical Therapy is not contracted with any HMO's. Unless you are pre-approved, we would ask that if you have an HMO or an out of network plan and wish to be seen at Larchmont Physical Therapy you need to pay a cash rate.

Medicare

Larchmont Physical Therapy is an approved Medicare provider. Please note, in order to receive treatment at an outpatient clinic you may not currently be under the care of home health services.

Workers' Compensation

In addition to having a referral from your physician, it will greatly expedite your care if you can provide us with as much information as possible regarding your workers' compensation carrier, your case manager, and any previous therapy that you have had to date on this injury.
