LPT Accepts PPO's, Medicare and Workers' Compensation Insurances

California insurance companies require that you have a prescription from a physician to be reimbursed for physical therapy. This prescription usually comes from an orthopedic surgeon but can be obtained from any doctor such as you primary care physician. If you choose not to see a physician, but do have a diagnosis, we can treat you at a cash rate. Please keep in mind that if you choose this path we may request you see a physician at some point to rule out more severe or unusual causes of your symptoms and/or pain.

HMO's/Out Of Network

Unfortunately, LPT is not contracted with any HMO's. Unless you are pre-appoved, we would ask that if you have an HMO or out of network plan and wish to been seen at LPT you need to pay a cash rate.

Blue Shield, Delta Health Systems, First Health, Medicare, Motion Picture, Screen Actor's Guild, Writer's Guild, Workers' Compensation

Medicare

Each of our therapists is an approved Medicare provider. You must not be currently under the care of home health services to be seen in an outpatient clinic.

Workers' Compensation

LPT accepts workers' compensation injuries. In addition to having a referral from your physician, it will greatly expedite your care if you can provide us with as much information as possible regarding your worker's compensation carrier, your case manager, and any previous therapy that you have had to date on this injury.
